Carlos T. Graveran to Lead BridgingLife as Assistant Vice President of Hospice and Palliative Care Services

BridgingLife, a non-profit organization dedicated to providing high-quality palliative and hospice services, today announced that Carlos T. Graveran will join the organization as assistant vice president of Hospice and Palliative Care Services, effective February 10. Previously known as Carroll Hospice, BridgingLife is a nationally recognized hospice program that has been providing end-of-life and palliative care services for nearly 40 years. Teams provide personalized high-quality, end-of-life care for individuals and support for their loved ones.

In his new role, Graveran will lead strategic and operational initiatives to support and grow BridgingLife—one of the leading hospice and palliative care programs in the mid-Atlantic region.“Carlos has extensive expertise in program development, operational excellence and community engagement that makes him uniquely suited to lead BridgingLife into a new era of growth and service,” says Garrett Hoover, president and chief operating officer at Carroll Hospital. Hoover is also senior vice president at LifeBridge Health. He adds, “We look forward to his leadership as we advance BridgingLife’s mission to deliver compassionate end-of-life care and palliative support for seriously ill individuals and their families.”Graveran most recently served as executive director of Frederick Health Hospice, where he expanded services and geographic reach while ensuring excellence in patient care, quality and financial performance. He has more than 30 years of experience in healthcare leadership, hospice management and strategic planning and is committed to enhancing community health services and fostering positive patient experiences.

Graveran is also deeply committed to community service and professional development, having held roles on several boards, including the Hospice and Palliative Care Network of Maryland and the Frederick County Veterans Advisory Council. His career accomplishments include numerous awards for excellence in business growth, leadership and diversity initiatives.

The cornerstone of BridgingLife is Dove House, a 14-bed facility located on the campus of Carroll Hospital. Dove House last year celebrated the completion of a three-year expansion that included the addition of seven new inpatient rooms, a new family lounge and nursing station, and a refresh of the original inpatient rooms and the administrative suite. Programs at Dove House include music therapy and pet therapy.

BridgingLife also provides hospice care in people’s homes, whether it’s a private residence or an assisted living facility, as well as on inpatient units at Sinai and Northwest hospitals, which are also both part of the LifeBridge Health system. Bereavement teams provide year-round grief support, helping thousands of families and community members each year. Additionally, BridgingLife offers palliative care services to support individuals living with serious illnesses and their family members. BridgingLife continues to add new programs as well, including expanding its support to veterans through a partnership with the Carroll County Veterans Independence Project and launching new death doula program to ensure no one spends their final moments alone. In the last fiscal year, BridgingLife took care of more than 2,400 hospice patients across its services, with an average daily census of 260 patients.

The teams also cared for more than 3,500 individuals receiving inpatient palliative care at Carroll, Northwest and Sinai hospitals BridgingLife is accredited by the Community Health Accreditation Program (CHAP). It is also Medicare certified and licensed by the State of Maryland. BridgingLife serves the Greater Baltimore area, Carroll and Frederick counties as well as southern Pennsylvania.
